Contact the Enfamil® Consumer Resource Center at 1-800-BABY123 (1-800-222-9123) Monday-Friday 8 am to 8 pm, Saturday 9 am to 5:30 pm Eastern Time to begin the myPurAmino enrollment process.
Step Two
Parent or Primary Caregiver
Access the enrollment form ↗ and complete the “To Be Completed By Parents” portion and provide a copy of insurance card (front and back).
Step Three
Healthcare Provider
Have your healthcare provider complete the “To be Completed by the Physician” portion of the enrollment form and send the completed form pages along with copies of your insurance card (front and back) to the email address or fax number provided by the Enfamil Consumer Resource Center.

How much does MyPurAmino cost to enroll?
MyPurAmino reimbursement support is provided at no cost to parents or primary caregivers and healthcare providers who enroll in the program.
What insurance plans is MyPurAmino able to work with?
PurAmino may be eligible for insurance coverage through Medicaid, Medi-Cal, TRICARE, and most commercial health insurance plans when prescribed.
If insurance may cover PurAmino, are there any out-of-pocket expenses?
Some insurance plans may require the deductible to be met before coverage begins, or they may require the parent or caregiver to pay a co-pay. MyPurAmino does not reimburse for out-of-pocket expenses including deductibles and co-pays.
What if my insurance plan denies a claim for PurAmino?
MyPurAmino can provide appeal process research and support by calling the parent's insurance plan to determine if there is an appeal process available in the case of a denied claim. If so, MyPurAmino will provide the next steps and help track the appeal once submitted.
